Once you have registered for our Faith Formation classes via our on-line form, please print out and complete the Faith Formation Medical Form.  Completed forms can be emailed to Mike Steich at [email protected] or dropped at the parish office in an envelope to the attention of Mike.  The information regarding your child’s particular needs will be kept confidential and only shared with the pastor, catechetical administrator, and child’s catechist.
